Register at Your Provider's Office
The preferred and easiest way to create your MyChart account is during your next appointment—just ask your care team to help you set it up right in the doctor’s office. It’s quick, simple, and gets you connected immediately.

If you were given an activation code at your last appointment, simply click the Login/Register button above to get started. Don’t worry if you don’t have an activation code—you can still register by providing a few details about yourself. Either way, getting connected is easy!

How do I sign up for MyChart?

The easiest way to create your MyChart account is during your next appointment—just ask your care team to help you set it up right in the doctor’s office. It’s quick, simple, and gets you connected immediately.

You can also register online using an activation code from a recent appointment, or by entering a few personal details if you don’t have one. If you’d rather handle it by phone, call the MyChart Hotline at 541.963.1800 and we can walk you through the setup.

Once registered, log in to your account online anytime at grh.org/mychart.

Will I be able to have access to my family’s electronic health record?

To set up proxy access to an adult family member’s electronic health record via MyChart:

  1. The person granting access must log into their own MyChart account and send an invitation to the person they want to give access to.
  2. The person receiving access must have their own MyChart account. Once they receive the invitation, they can log in and follow the steps to accept it.
  3. Need help? Contact the MyChart Hotline at 541.963.1800 for assistance or questions about proxy access setup.

To set up proxy access to a minor (under 18) family member’s electronic health record, please contact their provider or care team directly.
