Elgin Clinic Recognized for Top-Tier Primary Care with Tier 5 PCPCH Certification

Grande Ronde Hospital and Clinics is proud to announce that Elgin Clinic has earned Tier 5 certification through Oregon’s Patient-Centered Primary Care Home (PCPCH) program—the highest level of recognition available from the Oregon Health Authority.
This achievement places Elgin Clinic among an elite group of primary care providers in Oregon that go above and beyond in delivering accessible, coordinated, and patient-centered care. To earn Tier 5 status, clinics must meet all core requirements of the PCPCH model and demonstrate excellence in advanced care areas such as same-day access, behavioral health integration, proactive care planning, and equity-informed practices.

Elgin Clinic is the second GRH primary care site to earn Tier 5 certification. Union Clinic received the same recognition in July 2025. These designations highlight GRH’s commitment to providing exceptional, team-based care throughout Union County and surrounding rural communities.
