Grande Ronde Hospital and Clinics (GRH) has once again raised the bar in patient care, achieving a remarkably low 30-day readmission rate of just 6.85%. This represents a 40% reduction from the previous year's rate, which was already well below state and national standards. The hospital's focus on reducing readmissions continues to position it as a leader in rural health care, providing exceptional outcomes for its patients.
Strategic staffing
According to Wade Twilegar, Director of ICU and Inpatient Services, this significant reduction in readmissions is the result of deliberate staffing decisions and innovative care coordination.
"Our previous readmission rate was already exceptional, but through strategic efforts, we've reduced it by nearly 40%, which is a remarkable achievement," Twilegar says. "By strengthening our in-house hospitalist team and integrating telemedicine support, we ensure that each patient receives personalized care throughout their stay."
The hospital also employs three case managers who provide highly personalized care and help reduce the likelihood of readmission.
Expanded swing bed use
GRH's use of swing beds has been instrumental in maintaining these low readmission rates. By offering patients additional recovery support and enhancing coordination between physical therapists, occupational therapists and nursing staff, the hospital ensures that patients transitioning from skilled nursing facilities receive comprehensive care, which helps prevent unnecessary returns to the hospital.
Personalized follow-up care
Another critical component of GRH's success in keeping readmission rates low is its commitment to follow-up care. Every discharged patient receives a follow-up phone call within 48 hours, a practice that allows hospital managers to address any concerns and schedule follow-up appointments quickly. This proactive approach has been essential in reducing complications and ensuring patients' smooth recovery after they leave the hospital.
"Our ability to maintain such low readmission rates reflects the dedication of our team and the effectiveness of our patient care strategies," says Twilegar. "A 40% reduction is significant and reinforces our ongoing commitment to delivering the best possible care for our community."
