Home Health & Hospice
Our Work is Life.
The special people who are called to serve in our Home Health and Hospice care programs believe what they do goes beyond their efforts to heal the sick or comfort the grieving. We believe all our efforts are life-changing, life-affirming and miraculous. And we honor that.
Our Home Health Care team can help you stay in the comfort of your own home when you are sick, disabled or recovering from a stay in the hospital. Our highly skilled nurses, therapists, social workers and aides bring personalized health care right to your door. Our Home Health services include access to:
- Skilled nursing
- Physical, Speech, and occupational therapy
- Medical social workers
- Pain management
- Personal needs assistance, such as bathing
- Registered nurses on call 24 hours a day
We are available through referral, so please ask if your provider if you think our services could be helpful for you.
Our Hospice Care staff members are experienced and compassionate in caring for the terminally ill. Physical, social, spiritual and emotional care and support are key parts of the care we offer to patients and their families. Since Hospice care takes place in your home, you decide which services you want.
Services we provide for Hospice patients include:
- Home Health services listed above
- Palliative symptom management and bereavement care
- Spiritual care
- Assistance with everyday activities, such as bathing and dressing
- Respite care for our clients' regular caregivers
Many health plans help cover Hospice services, as do some local civic, charitable and religious organizations. Medicare and Medicaid may also cover Hospice for those who qualify.
