Description:

Requirements: Current Oregon LCSW in good standing. Must meet GRH credentialing criteria. Medical Social work experience preferred. Must have a dependable vehicle, valid Oregon driver’s license and proof of auto insurance. Provide coverage for hospice social worker, visit hospice patients and their families in the home setting. Must participate with the hospice care team in developing an interdisciplinary care plan. Assist with bereavement services.
